• All support staff and professional employees must wait three full years before adding a spouse or dependent(s) to insurance coverage
• All Act 93 employees may add themselves, a spouse and dependent(s) to insurance coverage upon becoming employed
• All employees may opt out of insurance coverage and will receive a stipend twice a year to reflect that waiver of available benefits
  • Benefit Waiver Form – OPT OUT
• All Full-Time Employees will receive Highmark PPO medical, Davis vision, Express Scripts prescriptions, and United Concordia dental
• Support Staff employees will receive PPO coverage
